Most people want to remain in their own home until they die, without consideration for their own safety or what is in their best interest.  We can help establish and keep your loved one as safe as possible, for as long as possible, in his or her own environment by...
  • Doing a risk assessment (for falls, poor nutrition, etc)
  • Evaluating and arranging for necessary home modifications, adaptations, and medical equipment
  • Arranging for and overseeing in-home caregivers
  • Medical monitoring (such as blood pressure, blood sugar levels, etc)
  • Medications set-up and monitoring
  • Making referrals to appropriate community resources (adult day care, home delivered meals, etc.)
  • Reviewing the financial situation to determine the most cost-effective support services available to meet his or her needs within the home
